What Is a Frenectomy?

A frenectomy is a minor surgical procedure that removes or releases a frenulum, the thin band of tissue that anchors the tongue to the floor of the mouth or the lip to the gums. When this tissue is unusually short, thick, or tightly attached, it restricts normal movement of the tongue or lip. This condition is often called tongue-tie or lip-tie, and while it is frequently discussed in the context of infant feeding, it does not always resolve on its own and can persist well into adulthood.

For adults, an untreated frenulum restriction can continue to affect speech clarity, oral hygiene, and comfort while eating. Removing or adjusting the tissue gives the tongue or lip a fuller range of motion, which often makes a noticeable difference almost immediately after healing.

Signs You May Need a Frenectomy

Many adults have lived with a tight frenulum for so long that they assume their symptoms are just how their mouth works. A few common indicators suggest it may be worth a consultation:

• Persistent difficulty pronouncing certain sounds, particularly those that require the tongue to reach the roof of the mouth
• Gum recession or a visible gap between the front teeth caused by a tight labial frenulum pulling on the gum tissue
• Trouble fully cleaning the teeth and gums because the tongue can’t move freely enough to help clear food debris
• Discomfort or fatigue while eating, especially with foods that require sustained chewing or tongue movement

Surgical and Laser Frenectomy Options

We offer two approaches to frenectomy, and the right one depends on the extent of the restriction and your overall oral health. A traditional surgical frenectomy uses a scalpel to remove the excess tissue, followed by sutures to close the site. A laser frenectomy uses a focused beam to release the tissue with minimal bleeding and often a shorter appointment time. 

Recent clinical research on laser-assisted lingual frenectomy paired with post-operative tongue exercises in adolescent and adult patients, published through the National Center for Biotechnology Information, points to this method as a way to improve tongue mobility and kinesthetic awareness in older patients, since restricted tongue movement affects speech well beyond childhood. We often recommend this procedure alongside pre-prosthetic soft tissue planning when a patient is also preparing for implants, dentures, or another restorative treatment.

Recovery After a Frenectomy

Most patients recover from a frenectomy within one to two weeks, with the majority of discomfort limited to the first several days. Soft foods, gentle rinsing, and specific tongue or lip stretching exercises help the tissue heal properly and prevent the frenulum from reattaching too tightly. We may also recommend platelet-rich fibrin therapy to support faster, more comfortable soft tissue healing after surgery. Following the post-operative instructions closely gives patients the best chance of a full, lasting improvement in tongue and lip mobility.

Beyond speech and comfort, a properly released frenulum plays a real role in long-term oral health. A tight lip frenulum that continually pulls on the gum tissue can accelerate recession and expose tooth roots, while a restricted tongue can make it harder to keep teeth and gums clean, raising the risk of decay and gum disease over time.
